D. SSAM SHS ENTRANCE SCHOLARSHIP - a scholarship grant that is given to Grade 11 qualified students from the
public schools of Marikina to have access to quality Catholic Benedictine education.
QUALIFICATIONS
• A Grade 10 Completer in a public school
• General Average of at least 88% with no grade in any subject below 85% in Grade 8 – 10
• Has passed the entrance examination and interviews of SSAM
• Attendance in a basic Catechism class before the opening of classes which will
be facilitated by SSAM
REQUIREMENTS
• PSA Birth Certificate
• Baptismal Certificate
• Certified true copy of Grade 8-9 Report Card
• Certified true copy of LATEST Report Card (Form 138/SF9) with the first quarter grades
  - A conduct grade of B+ or 85%
  - Numerical equivalent of letter grades
• Copy of the National Career Assessment Examination (NCAE) results (if
applicable)
• Four (4) pieces of recent 2" x 2" colored ID picture with the complete nametag
• Certificate of Good Moral Character (sealed in an envelope with signature across the flap or the school may directly
     email it to ssamregistrar@gmail.com)
• Accomplished SSAM Senior High School Information Sheet
• Accomplished Student Personal Data Sheet
• Accomplished SSAM Student Development Center Recommendation Form sealed in an envelope with a signature
across the flap
• Latest BIR Income Tax Return or any proof of income of parents
• Leadership Certification/s (Grade 8 – 10)
• Award Certification/s (Grade 8 – 10)
• Student Ranking Certification (Grade 9)
BENEFITS
• No fees – tuition, miscellaneous and special fees – to shoulder throughout the
duration of her
SHS education in the SHS Department
• No application and examination fees to pay for this scholarship program
• Free processing of her college application requirements
CONDITIONS FOR MAINTAINING THE SSAM SHS ENTRANCE SCHOLARSHIP
To keep the scholarship until
Grade Twelve, the applicant:
• complete the grade-level curriculum requirements at the end of the semester/school year;
• not have a grade lower than 88% in Theology and 85% in other subjects every quarter, and must have a final rating of
not lower than 88% in all subjects every end of the semester/school year;
• has an average conduct grade of A or A- every quarter, and no conduct grade lower than A- in any criterion, in any
quarter, every school year;
• actively involved in extra and co-curricular activities, with an ECA grade of not lower than 85% every quarter,
and a final rating of 88% every end of the semester/school year;
• not a recipient of any scholarship/benefit;
• attend a Basic Catechism program;
• update the Registrar and the batch sponsor regarding her quarterly performance.
Note: Those who cannot keep the academic and conduct requirements will be informed of the status of the scholarship
after the deliberation of the Scholarship Committee at the end of the school year
Those who will file a withdrawal/drop-out during the school year will be required to pay fifty percent (50%) of the school fees which are not covered by the SHS Voucher Program.
